Examples of insurance fraud:

• You receive an Explanation of Benefits statement for a service you did not receive.
• You have knowledge of someone using a Delta Dental identification card and providing false identification to the dentist.
• The treatment listed on the Explanation of Benefits statement does not match the treatment you received.
