What types of services are covered?


The Delta Dental PPO – Point of Service program features the perfect combination of access and savings. Individuals/Families choosing this option will access 3 classes of benefits and a $1,000 annual maximum per member per year. A $50 deductible applies to Class 2 services per person per year and a $200 deductible applies to Class 3 services per person per year.

Although you may see any licensed dentist, your out-of-pocket expenses will be lower when you utilize a Delta Dental PPO or Delta Dental Premier participating dentist.

Note: Current Delta Dental subscribers and persons currently covered by another dental carrier are not eligible to enroll.

Are there waiting periods?

Yes. To help keep your premiums low, there are waiting periods for Class 2 and 3 services. However, you are eligible for Class 1 services immediately.

  • No waiting for Class 1 Services - (Cleanings, exams, x-rays, etc)
  • 6 months for Class 2 Services - (Fillings, oral surgery, etc)
  • 12 months for Class 3 Services - (Crowns, bridges, dentures, etc)
How do I receive treatment?

You enroll and pay your fee, and after processing, you'll receive a Delta Dental PPO – Point of Service membership kit and ID card. On your effective date, simply present your card to any of the Delta Dental PPO or Delta Dental Premier participating dentists nationwide.

IMPORTANT NOTE: Your first month's premium is deducted immediately upon enrollment.

ABOUT YOUR EFFECTIVE DATE: When valid enrollment documentation and payment are received by DDOK on the 1st through the 20th of the month, coverage will be effective the first of the month immediately following. When valid enrollment documentation and payment is received by DDOK on the 21st through the last day of the month, coverage will be effective the first of the second month.

Example: If enrollment documentation/payment is received January 5, 2007—your coverage effective date is February 1, 2007; If enrollment documentation/payment is received January 23, 2007—coverage effective date is March 1, 2007.
