How much does this plan cost?

With Delta Dental PPO – Point of Service, you pay a low monthly fee and receive a comprehensive dental program ranging from routine preventive care to major restorative care.

Note: Current Delta Dental subscribers and persons currently covered by another dental carrier are not eligible to enroll.

Individual $35.00
Individual + 1 Dependent $64.40
Spouse or dependent child under age 19
Individual + Family $122.70
Spouse, if applicable and all dependent children under age 19


How do I pay?

The only payment option for the Delta Dental PPO – Point of Service product is by electronic funds transfers (drafts) from checking or savings accounts on a monthly basis. Initial premium will be drafted from your account immediately upon approval of your application. Subsequent drafts will be made on the 21st of each month and applied to the next month’s premium. This is to keep administrative costs (and your premiums) down. At this time debit or credit card payments are not available.

You are eligible immediately upon your effective date for Class 1 services (no waiting periods apply). If visiting a Delta Dental PPO or Premier participating provider, your claim will be filed for you by the dental office.


How do I receive treatment?

You enroll and pay our premium amount, and after processing, you'll receive a Delta Dental PPO – Point of Service member policy and ID card.

ABOUT YOUR EFFECTIVE DATE: When valid enrollment documentation and payment are received by DDOK on the 1st through the 20th of the month, coverage will be effective the first of the month immediately following. When valid enrollment documentation and payment are received by DDOK on the 21st through the last day of the month, coverage will be effective the first of the second month.

Example: If enrollment documentation/payment is received January 5, 2012 - your coverage effective date is February 1, 2012; If enrollment documentation/payment is received January 23, 2012 - coverage effective date is March 1, 2012.
